Social Worker, Mental Health

Advanced Center for Nursing and Rehabilitation is seeking a Social Worker to join its team. The main task of the social worker is to provide evaluation, psychotherapy and case management services to patients and families within our facility.

The Social Worker provides assessments, evaluations, treatment planning, short term- psychotherapy and social services to individuals, couples, families and groups as clinically determined. The Social Worker participates in clinical staff development, supervision and performance improvement activities as a member of an interdisciplinary team within a primary care medical setting. Most importantly, the Social Worker is a patient advocate and makes every effort to assure that the best interests of the patients are being met.
