Salary Range$61,500.00 - $91,875.00
OverviewThe Early Onset Cancer Program at Yale Cancer Center and Smilow Cancer Hospital focusing on broad goals related to younger patients' (ages 18-45) diagnosed with cancer: addressing psychosocial needs, developing novel clinical care models, enhancing clinical trials, expanding research, and raising community awareness about early onset cancers.
Reporting to the Early Onset Cancer Program Manager, the Early Onset Cancer Coordinator (EOC) will be a key member of the Early Onset CONNECT team, working collaboratively with our Early Onset Specialists (EOS) to support patients across a variety of needs including cultivating and tailoring informational and community resources, sharing educational and engagement opportunities, and connecting patients with research and support opportunities.
The EOC will reconnect and review needs with patients using a tailored needs assessment tool throughout the patient journey to best direct internal patient flow and support EOS caseloads. They will be a central liaison for internal and external community outreach and EO Cancer CONNECT information. They will also be responsible for administrative, operational, and programmatic tasks related to program implementation, maintenance, growth, and evaluation.
They will establish relationships and workflows to improve efficiency and effectiveness of the Early Onset CONNECT Program and be a partner with administrative and care teams, working closely with a variety of clinical and operational colleagues across the Smilow and YCC enterprise.
The Early Onset Cancer Coordinator will be responsible for independent decision making within a fast-paced environment and must remain flexible to rapidly shifting priorities. They are also expected to work well with key staff and leaders across several departments such as but not limited to breast, gastrointestinal, gynecologic, radiation, surgical, and research teams. The Early Onset Cancer Coordinator will be expected to advocate for patients and the program by identifying and assessing barriers, then recommending and/or facilitating collaborative and creative solutions to program leadership.
The Early Onset Cancer Coordinator will be expected to travel to Smilow Cancer Care sites across Connecticut and community outreach events as assigned. They will be expected to complete relevant training programs upon hire.
Skills And Abilities
- Strong ability to use empathetic probing skills to identify and address unique patient needs within family & cultural structures. Proven ability to rapidly engage with multiple patients throughout the workday while effectively meeting the needs of patients and program objectives.
- Ability to effectively communicate with a broad range of multidisciplinary stakeholders (i.e., patients, families, various medical providers, care network leadership, etc.). Proven ability to provide clear communication and critical information to internal team and external stakeholders and elicit feedback.
- Ability to consistently manage a patient caseload by developing and implementing strong organizational and time management skills that are flexible to rapidly changing priorities. Proven ability to schedule and coordinate program activities, develop action items, and use other strategies to keep work moving according to timeline.
- Ability to independently assess and initiate action in real time using sound judgment and patient-centered advocacy at both program and patient levels. Ability to consistently develop and utilize program-related documents, and other innovative solutions to reduce patient barriers.
