The Clinical Social Worker II is a licensed, experienced professional who is the clinical expert in the assessment and treatment of patients, families and social work consultation to the interdisciplinary teams in a variety of inpatient and ambulatory settings in the Yale New Haven Health System. The Clinical Social Worker II consistently performs beyond required job specifications, serves as a role model and mentors colleagues as evidenced by progression through the social work advancement program.

As the clinical expert, the scope of practice includes, but is not limited to, diagnostic and teaching/mentoring. The clinical social worker will work collaboratively with members of the health care teams throughout the organization.

• 9.Provide information, consultation and professional support to interdisciplinary team members and colleagues within the hospital and community.
• 10.Facilitate and/or participate in interdisciplinary rounds and other patient related discussions to promote integrated care and improve patient outcomes.
• 11.Initiate and maintain contact with managed care representatives to obtain pre-certification and ongoing authorization, to ensure reimbursement for services, as appropriate. Submit charges for services rendered as appropriate.
• 12.Demonstrate initiative and leadership in developing services and programs to enhance patient and family care, including development of evidence-based practices.
• 13.Contribute to individual, team, departmental and organizational quality assurance and performance improvement.
• 14.Document timely, accurate and clinically relevant entries of social work activities in the electronic medical record.
• 15.Navigate systems and provide advocacy for patients and families as indicated.
• 16.Orient, precept, mentor and supervise students/trainees, junior colleagues and new employees.

Master's Degree from an accredited school of social work.
