SummaryThe Nurse Coordinator is responsible for ensuring high quality patient care that is coordinated, efficient, and supports desired clinical and financial outcomes for patients in the urology service line. In collaboration with the healthcare team, the Nurse Coordinator develops and utilizes critical pathways to achieve specific patient outcomes and ensure appropriate use of resources within designated fiscal time frames. The role involves ongoing evaluation of practice patterns and systems, improving patient care and operational efficiency, and identifying and addressing trends and barriers in care.
Responsibilities- Establish a system for coordinating the care of patients throughout the continuum of care and linking them to resources to maximize quality and safety.
- Collaborate with inpatient nursing staff for coordination of complex admissions and discharges and with referring physicians and community outreach centers to facilitate urologic services.
- Review health‑care information with the team, monitor patients’ progress, and ensure adherence to clinical pathways and treatment plans.
- Facilitate communication within the health‑care team and with primary care providers and other disciplines to coordinate patient progress and intervention sequencing.
- Optimize the efficiency of hospital systems that impact quality care and streamline scheduling and access to tests and consultations.
- Intervene to correct delays and address barriers to care, identify trends and recurring problems, and collaborate with the team to resolve them.
- Ensure patients meet eligibility requirements and clinical needs for program services, initiate appropriate referrals, and manage abnormal test results within established policies.
- Advocate for patients by linking them with institutional or community resources, securing necessary referrals, and developing new resources where gaps exist.
- Act as a liaison among healthcare providers, including physicians, social workers, and substance‑abuse resources, to ensure coordinated patient care.
- Educate patients and families regarding services provided by the program and maintain open communication among all team members.
- Serve as an advocate for patients and uphold ethical and legal standards in patient care.
- Education:
RN, BSN required. - Experience:
Minimum of five (5) years of relevant clinical experience with at least two (2) years in a specialty area.
Clinical credibility with physicians and nurses; interpersonal skills to negotiate scarce resources and broker with outside agencies; excellent communication skills across multiple disciplines; ability to process complex problems, identify key components, and develop resolution plans; demonstrated skills in clinical management of specific patient populations; ability to provide clinical and financial education to colleagues, patients, and families.
AccountabilityThe position is accountable for coordinating and managing the care of urologic patients, ensuring that patients progress in accordance with established clinical and financial outcomes, and intervening to correct delays. The role also involves analyzing variances, implementing practice changes, and ensuring high‑quality, family‑centered care.
Licensure / CertificationCT RN Licensure.
