Position: Ambulatory Neurology/Epilepsy Advanced Practice Provider ( Full Time)

Position Summary
  • Provide comprehensive, patient‑centered care to individuals with epilepsy in a busy ambulatory neurology practice.
  • Manage complex epilepsy patients in close collaboration with physician partners and the multidisciplinary care team.
  • Deliver high‑quality outpatient neurological care with a strong focus on education, continuity, and long‑term management.
Position Highlights

Key Responsibilities
  • Evaluate, diagnose, and manage patients with epilepsy in an outpatient setting.
  • Develop and implement treatment plans in collaboration with neurology physicians.
  • Monitor patient progress, adjust therapies, and provide ongoing education and support.
  • Document patient encounters accurately and efficiently in the electronic medical record.
  • Participate in team‑based care coordination to ensure optimal patient outcomes.
Responsibilities
  • Clinical Care — Provide assessment, triage, therapeutic management, obtain medical histories, perform physical examinations, review/order diagnostic tests, formulate and manage treatment plans, perform procedures, prescribe treatment and medications in accordance with institutional and state guidelines, assist as first or second operative assistant, perform other clinical duties as per department goals.
  • Patient Education & Psychosocial Support — Educate patients/families on care and recommended resources, provide psychosocial support as part of multidisciplinary team.
  • Information Management — Ensure appropriate documentation within hospital and regulatory requirements, protect information from unauthorized access.
  • Quality Management — Demonstrate high standards of moral and ethical behavior, ensure compliance with patient‑related healthcare regulatory requirements, provide professional competence.
  • Professional Development — Engage in projects that maintain and advance professional competency, serve as clinical resource and mentor, collaborate on research and quality improvement projects, maintain employee requirements, serve as preceptor as appropriate.
Qualifications
  • Licensed Nurse Practitioner or Physician Assistant (or eligible for licensure).
  • New graduates are welcome; robust onboarding and structured orientation provided.
  • Prior experience in neurology or epilepsy preferred for experienced APPs.
  • Strong clinical judgment, communication skills, and commitment to patient‑centered care.
Education
  • Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
Experience
  • Clinical working experience as an NP/PA in the specialty/service line is preferred but not required.
  • APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS hospital and go through the Medical Staff credentialing process; evidence of current competence must be provided.
Licensure
  • Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certified; when indicated for position profile DEA License.
  • Pediatric position profile PALS, when indicated for neonatal care NRP Certification.
  • Additional licensure, certifications and/or continuing education credits may be required during employment based on position and area of specialty; OR Connecticut State Registered Nurse License, Connecticut State Nurse Practitioner License, Board specialty certification as appropriate.
