The Home Infusion RN is a member of the YNHH Home Infusion Therapy team. Responsible for the delivery of high‑quality care as it relates to delivery of infusion therapy in the home or alternate setting. Provides education to patient and/or caregiver on proper handling, storage and administration of your infusion medications as well as signs and symptoms of access device complications.
Coordinates patient care and maintains communication with the physician, pharmacy team as well as nursing agency(s).
Nursing diploma or associate degree in nursing. Bachelor's degree in nursing preferred. CRNI certification a plus.
ExperienceThree years of IV related experience. Previous experience in the home care or home infusion setting is desirable. Case management experience a plus.
LicensureRegistered Nurse with a valid (or eligible) Connecticut licensure required.
Special Skills- Experienced in the care of peripheral and central lines.
- Proficient in troubleshooting potential problems in IV therapy.
- Knowledge of medications, indications, dosage ranges, side effects, and potential toxicities.
- Ability to work independently.
- Excellent customer service skills (both verbal and written) and effective interpersonal skills.
- Proficient computer skills.
Must be capable of lifting 20 lbs, standing, walking, and bending.
