Yale New Haven Health is looking for a Total Joint Replacement (TJR) Nurse Navigator to facilitate exceptional patient care for total joint patients. This role includes improving patient preparedness through education, coordinating care with multidisciplinary teams, and optimizing patient experiences before and after surgery.
The ideal candidate will possess a Bachelor’s Degree in Nursing and have experience within Orthopaedic nursing. Skills in communication, critical thinking, and proficiency in EPIC are essential.
