Under the direction of the Manager, Director of Nursing, and Physician Director, the Nurse Coordinator works collaboratively with physician leaders, physicians, Advanced Practice Providers (APP) and staff of the Neurosciences service line to ensure goals and objectives are successfully met. The primary role of the Nurse Coordinator is to maintain communication with patients, families, staff, and providers in order to ensure coordination of patient care across the continuum.
Responsibilities- Manages incoming referrals, coordinates initial patient contact, and complies list of presented patients in collaboration with attending physicians, fellows, and Advanced Practice Provider (APP's).
- Assists in the coordination of care in the Neurosciences service line.
- Creates and supports a culture and environment of safety.
- Participates in enhancing the professional development of the nursing staff and other health care providers involved in the care of Neurology/Neurosurgery patients.
Bachelor's Degree in Nursing required. Connecticut RN license
ExperienceA minimum of three (3) years experience in a combination of the following will be considered: adult acute care inpatient services, ambulatory or critical care.
LicensureRN license in the State of Connecticut
Special SkillsBaseline knowledge of neuroscience disease states. Excellent collaborative, communication, and interpersonal skills required. Ability to triage non-urgent and emergent situations.
