HC - Nursing (Ambulatory Local + Per Diem)

WCS Healthcare is seeking an experienced and compassionate Lead RN Care Coordinator to oversee the delivery of comprehensive health services for children in New Haven, CT.

Job Function:
  • Day Shift.
  • 8:30 AM – 5:00 PM.
  • Oversee and coordinate healthcare services for children, ensuring timely and appropriate medical, dental, and behavioral health support
  • Supervise and mentor care coordination staff, providing day-to-day guidance and oversight
  • Develop and deliver staff training on healthcare protocols, public health practices, and compliance standards
  • Coordinate interpretation and translation services to ensure equitable access to care
  • Support care transitions by coordinating post-program or post-release healthcare services
  • Collaborate with healthcare providers, agencies, and community resources to ensure continuity of care
  • Monitor compliance with healthcare regulations and organizational policies
  • Maintain accurate, timely, and compliant medical documentation
  • Participate in interdisciplinary meetings to support holistic care planning
Job Requirements:

Job Requirements:

  • Active Registered Nurse (RN) license in Connecticut
  • Bachelor’s degree in Nursing or higher
  • Minimum of 2+ years of clinical experience (pediatrics, community health, or care coordination strongly preferred)
  • Strong knowledge of care management principles and community resources
  • Excellent communication and organizational skills
  • Ability to work independently while collaborating within a team environment
  • Prior supervisory or leadership experience in a healthcare environment
  • Bilingual in English and Spanish (required)
