The Registered Nurse After‑Hours Nurse serves as a member of the Interdisciplinary Team (IDT) and is primarily responsible for triaging issues, admission and resumption of care visits, providing compassionate, comprehensive care of patients, developing the patient plan of care, coordinating with the patient’s provider and other disciplines for the provision of services, and safely delivering patient care, enhancing the quality of life during end‑of‑life care.
ResponsibilitiesNursing Diploma or Associates Degree in Nursing required. Bachelors in Nursing preferred.
ExperienceMinimum of two (2) years of nursing experience preferred. Previous home care/hospice experience preferred.
LicensureRegistered Nurse with a valid (or eligible) Connecticut license required. A valid driver’s license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.
SpecialSkills
Understanding of Medicare certification and CMS data collection tool documentation experience. Strong organizational skills and the ability to work independently. Strong computer skills and the ability to learn new software. Ability to manage a variety of assignments while establishing priorities and ensuring that necessary duties are completed.
Physical DemandAbility to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials and communicable diseases.
Valid CT RN license, valid driver’s license. Home Health and Hospice experience required.
Hours:
4:30pm‑8am. 7days on/7 days off on call.
Phone triage, admissions, pronouncements.
