Position: Hospice Registered Nurse- After Hours

Overview

The Registered Nurse After‑Hours Nurse serves as a member of the Interdisciplinary Team (IDT) and is primarily responsible for triaging issues, admission and resumption of care visits, providing compassionate, comprehensive care of patients, developing the patient plan of care, coordinating with the patient’s provider and other disciplines for the provision of services, and safely delivering patient care, enhancing the quality of life during end‑of‑life care.

Responsibilities
  • Coordinates all patient care activities after hours, on weekends, and holidays, including triage of patient calls, patient visits, and referral coordination.
  • Assesses/reassesses patients and establishes plans of care that are patient‑specific.
  • Collaborates with providers, other disciplines, and communicates all changes promptly.
  • Educates patients/caregivers regarding hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary.
  • Completes all documentation in the EMR within 24 hours. Must include changes in condition, follow‑up with the provider or other disciplines, with supporting documentation.
  • Oversight of LPNs and Home Health Aides.
  • Reviews and updates the POC per regulatory standards.
  • Schedules patients independently according to the individual plan of care.
  • Thorough and accurate CMS data collection tool documentation.
  • Builds and nurtures relationships with referral sources, physicians, and facility staff to enhance visibility and trust in hospice services.
  • Attends all staff meetings, in‑services, and other required meetings. Compliance with all mandatory requirements.
  • Participates in after‑hours, weekend, and holiday coverage as needed.
  • Follows policies and procedures.
  • Understands and exemplifies hospice philosophy, values, beliefs, and team concepts in all job‑related roles.
  • Submits reports as required regarding extended hours.
  • Maintains communication with the answering service as necessary.
  • Coordinates and manages the care of patients receiving services in their home.
  • Participates in case management conferences and documents per guidelines.
  • May perform other duties as assigned.
  • Qualifications Education

    Nursing Diploma or Associates Degree in Nursing required. Bachelors in Nursing preferred.

    Experience

    Minimum of two (2) years of nursing experience preferred. Previous home care/hospice experience preferred.

    Licensure

    Registered Nurse with a valid (or eligible) Connecticut license required. A valid driver’s license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.

    Special

    Skills

    Understanding of Medicare certification and CMS data collection tool documentation experience. Strong organizational skills and the ability to work independently. Strong computer skills and the ability to learn new software. Ability to manage a variety of assignments while establishing priorities and ensuring that necessary duties are completed.

    Physical Demand

    Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials and communicable diseases.

    Valid CT RN license, valid driver’s license. Home Health and Hospice experience required.

    Hours:

    4:30pm‑8am. 7days on/7 days off on call.

    Phone triage, admissions, pronouncements.
