In-Home Health - NP​/PA; Part Time - Hartford, Connecticut

Overview

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. The clinician will visit patients in their homes, conducting evaluations lasting about 45 minutes, reviewing medication and medical history, performing a physical assessment, and ordering optional diagnostic tests as directed by the patient’s health plan. The role focuses on creating meaningful relationships and linking patients to appropriate care.

• Conduct in‑home health evaluations, including medication and medical history reviews, physical examinations, and optional diagnostic tests (e.g., diabetic eye exams, spirometry).
• Build and maintain trusted relationships with patients to improve health outcomes.
• Coordinate with Signify Health colleagues and health plans to provide appropriate diagnostic and preventive services.
• Travel locally and within licensed states as required and conduct virtual visits as needed.
• Use an iPad and an intuitive clinical workflow for each evaluation; prescribing, ordering lab tests, or altering treatment regimens is not part of this role.

• Active, unrestricted NP or PA license(s) in the coverage area.
• Board certification (required) and multi‑state licenses (preferred).
• Ability to cover the primary location:
  Hartford and surrounding area.

• Previous experience providing in‑home or community health services.
• Experience with diagnostic and preventive service products such as spirometry.

Part‑time position with an anticipated weekly hours of 29.5. Pay range: $46.03 – $99.14 per hour, based on experience, education, and geography.
