Responsibilities
- Provides comprehensive assessment, triage, and management of patient and family needs throughout the disease process as part of an interdisciplinary healthcare team in inpatient, ambulatory, and outpatient settings.
- Collaborates with supervising or collaborating physicians per state laws, hospital regulations, policies, clinical pathways, and protocols; may provide autonomous, team‑based care for designated patient populations (depending on specialty and scope).
- Obtains medical histories and performs physical examinations.
- Orders, reviews, and interprets diagnostic and therapeutic tests, including imaging.
- Develops and manages patient treatment plans.
- Prescribes medications and treatments consistent with institutional and state guidelines.
- Performs clinical procedures within professional scope and competency, including assistance in surgical procedures when credentialed.
- Responds to emergency situations in alignment with YNHHS policies.
- Educates patients and families about diagnoses, treatment plans, medications, side effects, and available resources; provides admission instructions, ongoing care guidance, and discharge planning.
- Provides psychosocial support to patients, families, and significant others while maintaining ethical and professional standards of care.
- Ensures accurate, timely, and complete documentation in compliance with hospital, legal, and regulatory requirements; safeguards patient information.
- Maintains compliance with all applicable healthcare regulations, standards, and best‑practice recommendations; promotes patient safety and quality outcomes.
- Engages in ongoing professional development to maintain and enhance clinical competency; serves as a clinical resource and educator; participates in orientation and educational initiatives; reports safety events; maintains licensure and certification; precepts students; collaborates on research, quality improvement, performance improvement initiatives, and protocol development.
- Graduate from an accredited Nurse Practitioner or Physician Assistant Program.
- Clinical working experience as an NP/PA in the specialty/service line is preferred but not required.
- APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS hospital and go through the Medical Staff credentialing process.
- Active PA Certification (NCCPA) and Connecticut State Physician Assistant License; ACLS/BLS Certified; DEA License when indicated.
- For pediatric positions: PALS and NRP Certification as appropriate.
- Additional licensure, certifications, and continuing education credits may be required based on position and specialty.
