About This Role
As a Registered Nurse in the Acute Care for Older Adults Medicine with Telemetry unit, you will provide high‑quality, patient‑centered care while collaborating with healthcare teams to ensure optimal outcomes for patients.
LocationNew Haven, CT
What You’ll Do- Establish and maintain therapeutic relationships with patients and families.
- Communicate and collaborate with patients, families, and healthcare team members.
- Respect patients’ values, preferences, and expressed needs.
- Collect and analyze assessment data to determine diagnoses.
- Develop and modify individualized care plans.
- Coordinate care delivery and transitions in care.
- Promote health and a safe environment through teaching.
- Evaluate progress toward attainment of health outcomes.
- Education: Graduation from an accredited school of nursing; BSN preferred.
- Experience: Registered Nurse is the foundational job description for all RNs.
- Licenses &
Certifications:- Licensed and currently registered nurse in the State of Connecticut.
- Current BCLS certification or must obtain within one month of hire.
- Clinical specialty certification highly encouraged.
