The Licensed Practical Nurse I provides care to patients and is responsible for performing designated tasks and responsibilities under the direction of a Registered Nurse, with a focus on supportive and restorative care, health counseling and teaching. The LPN I demonstrates clinical judgment by applying problem‑solving techniques and integrating skills and knowledge into clinical practice.

Responsibilities
  • Provides direct patient care with the established model of nursing as evidenced by the following:
    • Documents admission information under supervision of the Registered Nurse
  • Collaborates with the Registered Nurse and other members of the Health Care Team.
  • Verifies, prepares, and administers medication to all assigned patients to ensure safe medication delivery and positive outcomes.
    • Clarifies any questions regarding physician prescription orders immediately to the Registered Nurse 100% of the time.
  • Participates in team building activities that contribute to patient care, staff education, and personal development.
    • Participates in interdisciplinary conferences.
  • Maintains clinical competency to ensure that practice is evidence‑based.
    • Attends all mandatory and continuing educational classes.
Qualifications

Education:

Graduated from an accredited LPN program. Current license in the State of Connecticut.

Experience:

Prefer experience in a clinical procedural area. Must complete an approved medication course and forty (40) hours of supervised medication administration within one year of employment with YNHHS. Must also demonstrate competency in designated hospital and unit.

Physical Demands:
Requires prolonged standing; walking; use of sight, hearing and touch; and moving/lifting patients of varying weights; CPR.
