Job Purpose

Nurses provide patient care and leadership support to the Care Team by collaborating with team members and performing high-quality, cost-effective nursing care using the nursing process: assessment, diagnosis, planning, implementation, and evaluation. Nurses promote health maintenance and restoration of patients’ well‑being in collaboration with all team members, guided by FHCHC policies, standards of care, and mission.

Duties and Responsibilities
  • Provide direct patient care in clinic or via telephone, independently carrying out assignments.
  • Collaborate and lead within the Patient Care Team.
  • Supervise Licensed Practical Nurses (LPN) and Medical Assistants (MA), delegate tasks, and assist in developing patients’ plans of care.
  • Document in the EMR, process forms, and perform follow‑up phone calls related to the patient’s plan of care.
  • Participate in care coordination, pod huddles, team and departmental meetings, and oversee clinic flow to maximize efficiency.
  • Address patient complaints or concerns, document them, and consult Nursing Management with risk‑management issues.
  • Assist the Nurse Manager with daily schedules, coverage solutions, and coverage decisions.
  • Respond to emergency situations per FHCHC protocols and perform monthly checks on emergency equipment.
  • Order medications per FHCHC policies, maintain medication inventory, and stock the medication room.
  • Conduct phone/electronic follow‑up, place calls to patients, assist in completing patient forms, and follow all FHCHC EHR documentation policies.
  • Coordinate specialty services, manage anticoagulation therapy, and work closely with VNA nurses for referrals.
  • Assist with initial patient assessments, document history, medications, allergies, and collaborate on nursing triage.
  • Administer medications, vaccines, and perform clinical procedures per standing or clinician orders.
  • Perform medication reconciliation, conduct assessments and interventions identified in panel‑management meetings, and collaborate with LPN/MA on care planning.
  • Provide health education by phone or after office visits, review the After‑Visit Summary with patients as needed, and document all teaching outcomes.
Primary Objectives
  • Deliver excellent nursing care in a patient‑centered medical home model.
  • Demonstrate nursing leadership within the primary care team and the broader FHCHC community.
  • Contribute to nursing, medical care, and quality improvement at FHCHC and the community.
Supervision (Team Specific)
  • Supervise LPN and MA with general oversight.
  • Assist in completing annual clinical competencies and provide feedback to the Nurse Manager on performance reviews.
  • Provide coaching, counseling, and support to direct reports.
  • Demonstrate good patient‑care management principles and communication skills.
Documentation, Forms Management & Phone Calls
  • Perform phone or electronic follow‑up messages.
  • Place calls to patients for questions, assessments, or lab result reviews, as directed by the PCP.
  • Assist in completing patient forms and follow all FHCHC EHR documentation policies.
Care Coordination
  • Manage anticoagulation therapy for Coumadin patients.
  • Collaborate with VNA nurses on new referrals and ongoing patient care.
  • Coordinate specialty services internally and externally.
  • Provide feedback to the care team regarding care‑coordination status.
Patient Assessment, Diagnosis, Planning, Implementation & Evaluation
  • Assist with initial patient documentation of medical history, medications, and nursing process elements.
  • Discuss allergies and other relevant details with the PCP.
  • Conduct nursing triage on phone and in office.
  • Administer medications, vaccines, and perform clinical procedures per orders.
  • Perform medication reconciliation.
  • Execute assessments and interventions identified in panel‑maintenance meetings.
  • Collaborate with LPN/MA in developing the patient’s plan of care.
Patient Education
  • Provide health education by phone or after office visits.
  • Review the After‑Visit Summary with patients as needed.
  • Document all patient or family teaching and outcomes.
