• New Haven Territory
• Saturday to Monday OR Friday to Sunday

• Work 3 Days Only (typically Friday through Sunday OR Saturday through Monday)

You’ll bring high-quality, compassionate care directly into patients’ homes—when they need it most.

• Manage a weekend caseload within your local territory
• Conduct skilled nursing visits, assessments, and follow-ups
• Develop and adjust personalized care plans
• Partner with physicians and care teams to ensure continuity of care
• Educate patients and families to promote independence and safety at home

• Active RN license (CT)
• 1+ year of nursing experience (Home health preferred not required)
• Willingness to travel locally for patient visits
• Valid driver's license, reliable vehicle & insurance
• Live within the service area
• Lift, pull, and push 50-100lbs
