LPN, LPN​/LVN, RN Nurse

Hartford Healthcare seeks a compassionate Licensed Practical Nurse to provide patient-centered care at our Trumbull and Monroe locations. In this role, you'll support RNs and providers with assessments, medication administration, treatments, and patient education across a range of primary and specialty services. Responsibilities

• Provide direct patient care under RN and provider supervision in outpatient and clinic settings
• Administer medications, treatments, and injections following established protocols
• Monitor, document, and report patient status, vitals, and responses to care in the EMRAssist with exams, procedures, and coordination of patient flow for Trumbull and Monroe locations
• Educate patients and families on care plans, medications, and follow-up instructions
• Collaborate with interdisciplinary team members to ensure safe, high-quality care
• Maintain infection control, safety, and regulatory compliance standards
• Support care coordination including scheduling, referrals, and discharge planning
