Role Description

This is a full-time, on-site role located in New Haven, CT, for a Family Case Manager. The Family Case Manager will be responsible for conducting comprehensive case management services, ensuring support to families and individuals in need of assistance. Daily responsibilities will include providing guidance in family services, coordinating resources, and addressing critical social work matters. The role involves creating personalized action plans, collaborating with community partners, and maintaining detailed documentation of client progress.

Qualifications
  • Strong foundation in Social Work and Family Services to ensure effective and empathetic client support
  • Experience or knowledge in community service and resources, and working with vulnerable family structures
  • Proven skills in Case Management and delivery of Case Management Services
  • Excellent problem-solving, organizational, and communication skills
  • Ability to prioritize and meet deadlines in a community-centered environment
  • Bachelor’s degree in Social Work, Human Services, or a related field (preference)
  • Fluency in Spanish
  • Previous experience working with diverse and underserved communities is an asset
