JobTitle and Summary
Case Manager – Conducts the intake/admission and assessments; develops and implements the service plan; provides case management and re‑entry/transition planning; primary contact for referral source; connect clients to community resources in collaboration with referring/assigned officer, liaison with various systems (court, social services, treatment providers, etc.); may need to attend court; engages and develops collaborations with community agencies; develops and submits required reports to referring agent and/or court;
coordinates and/or facilitates groups and activities. Assist with screenings as needed. May transport clients, based on programmatic needs. Must possess a valid driver's license. Expected to work some evenings and weekends.
- Engage clients using Motivational Interviewing techniques in day-to-day interactions.
- Delivers gender responsive and culturally competent services (i.e. case management sessions, assessments).
- Schedule case management sessions.
- Provides case management services in accordance with the Carey Guides.
- Conducts approved assessments on clients. All assessments must be audio taped and submitted for quality assurance.
- Refer clients to programs and services based on client's risk, needs, strengths, and responsivity factors and court‑ordered mandates.
- Develop Individualized Service Plans with respect to assessment findings and court‑ordered mandates.
- Facilitate instructional and intervention group services.
- Ensure security and confidentiality and uniformity of content and management for all client records.
- Provide timely and accurate file and database documentation. Keep records up to date at ALL times.
- Enter accurate data into Contractor Data Collection System.
- Act as a liaison to the court, referral sources, community resources and organizations.
- Represent program and Agency in a professional manner.
- Remain current on research and literature for "best practices" for working with adult justice populations.
- Maintain ongoing communication with referral agent.
- Prepare court reports, monthly reports and discharge reports that are accurate and timely.
- Must attend trainings as set for by CSSD (Court Support Services Division) and meet with a quality assurance coach to review assessments and case management sessions monthly.
- Participate in quality assurance activities that include audio/video taping interactions with clients, submitting tapes, and participating in feedback sessions with QA coaches.
- Implement behavior motivation model.
- Model pro‑social behaviors and serve as a role model to clients and staff.
- Maintain professional relationships with clients.
- Participate in multidisciplinary treatment team meetings weekly.
- Candidate will report directly to Program Director or his/her designee.
- Bachelor's Degree and 3 years' experience in case management and a minimum of 1 year of client supervision. Education may be substituted for experience on a year‑for‑year basis.
