About this Position

Community Resources for Justice (CRJ) is a nonprofit organization dedicated to changing lives and strengthening communities through direct service, advocacy, and innovation. CRJ operates across four core service lines:
Community Strategies, Reentry Services, Crime & Justice Institute, and Family Reentry.

Job Summary

The Case Manager is responsible for managing the outpatient caseload, monitoring weekly activities and assisting clients with employment, housing, transitional, and other discharge planning requirements.

Essential Duties and Responsibilities
  • Develops and monitors Individual Program Plans for clients on caseload.
  • Meets regularly with each client to discuss progress, program requirements, personal goals, and compliance; reviews weekly schedules and engages clients in treatment.
  • Maintains ongoing communication with referral providers and family members.
  • Provides timely information to appropriate personnel for coordination of services and reporting purposes.
  • Completes all required paperwork for each client on caseload.
  • Attends necessary meetings, conferences, and training within CRJ and with outside providers.
  • Receives referrals from the referral source and schedules clients within two business days; if intake cannot be scheduled within two weeks, informs the referral source and remedies the situation.
  • Communicates with clients about all appointments, ensuring the appointment reminder system is up to date to reduce no‑show rates.
  • Assesses client needs during meetings and reports updates to the clinical team as needed.
  • Coordinates clinical case management services with community-based providers in housing, employment, and eligibility for Medicaid, Medicare, SSI, SSDI, and other benefits; makes referrals for additional services and supports.
  • Meets weekly with the Clinical Director for supervision and problem‑solving of cases.

Skills and Abilities
  • Exceptional organizational skills with a keen eye for detail and accuracy.
  • Strong communication skills and ability to collaborate with staff.
  • Effective time‑management and prioritization skills.
  • Excellent typing skills and proficiency with Windows, Microsoft Office, and Excel.
Required

Education and Experience
  • BS/BA from an accredited college or university in Criminal Justice, Human Services, or a related field.
Preferred

Education and Experience
  • One year of case‑management experience preferred.
  • Experience with a correctional population preferred.
Other Duties

Other duties may be assigned as needed and may change over time with or without notice based on business needs.

Pay Range

23 – 27 USD per hour (US).
