Director of Social Services

Location: New Haven, CT

Employment Type: Full-time

Responsibilities
  • Implement and document a plan of care related to residents' social and emotional needs in the electronic medical record.
  • Collaborate with nursing, MDS, and rehabilitation departments to facilitate discharge plans for residents.
  • Inform and educate staff, residents, families, and responsible parties about residents' personal and property rights and health service rights.
  • Maintain updated resources for residents and families, including community resources and service agencies.
  • Maintain a record of all reported concerns, assist in resolving them, and ensure appropriate follow-up and documentation.
  • Be easily accessible to residents, families, and staff.
  • Provide timely referrals to appropriate agencies for assistance (e.g., conservatorship applications, Medicaid applications).
Qualifications
  • Bachelor's or Master’s degree from an accredited program in Social Work or related field.
  • Previous experience in a long‑term care facility and familiarity with current Requirements of Participation regulations.
  • Knowledge of admissions and discharge procedures, residents' rights, and documentation requirements in accordance with current regulations and guidelines.
  • Attention to detail, good follow‑through skills, and ability to prioritize multiple tasks.
  • Innovative with the ability to find creative ways to serve diverse resident needs and maintain a customer focus.
  • Vehicle and valid driver’s license.
  • Demonstrates cooperation and ability to work as a team.
  • Patience, tact, enthusiasm, and a positive attitude toward residents and their support networks.
  • Ability to remain calm in stressful situations.
  • Ability to communicate with residents, staff, and family members professionally.
