The Connection is seeking a Case Manager 2 in New Haven, CT to perform intake assessments, risk/needs evaluations, and to develop and monitor individual service plans with a focus on community re-entry after incarceration.
You will manage a caseload within an adult services team, coordinate with treatment providers, and support residents’ recovery from mental illness, substance use, and homelessness.
