Delta-T Group North Jersey, Inc. seeks a Rapid Rehousing Case Manager to serve clients in the New Haven, CT area. This full-time role focuses on assessing needs, assisting with housing vouchers, and securing apartments for clients experiencing homelessness.
You will document eligibility, coordinate referrals for mental health and addiction services, support resume writing and job search, and track client progress even after housing is secured.
Hours:
Mon-Fri 8:30–4:30.
