$15,000 Sign‑On Bonus or Student Loan Assistance is available. MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury, or surgery, allowing them to heal in the comfort of their own homes.

• Map out and oversee the patient’s recovery from illness, injury, or surgery at home, providing advanced clinical care including patient assessment, medication management, and education for patients and families.
• Coordinate, implement, and continually evaluate the home‑care needs of patients, reviewing insurance information and ensuring all care activities are documented and communicated to family and professional caregivers.
• Ensure findings are shared with relevant parties, supporting agency‑established financial goals.
• Demonstrate expert knowledge unique to community health nursing and maintain excellent communication, documentation, and time‑management capabilities.

• Current NYS RN license and registration.
• Minimum of one year acute medical‑surgical nursing experience.
• Bachelor’s degree in nursing preferred.
• CHHA experience preferred.
• Valid driver’s license and access to a car required.
• Strong ability to solve problems independently and work effectively with an integrated team.
• Excellent clinical assessment skills and expert knowledge unique to community health nursing.
