Salary: $20.00-$23.00/hour

• Conduct client intakes for human services programs.
• Connect individuals and families with community resources, referrals, and support services.
• Communicate with clients, healthcare providers, and community partners.
• Attend community outreach events and represent program services.
• Assist clients with maintaining eligibility for government programs and accessing needed resources.
• Support documentation and follow-up activities related to client services.
• Experience conducting client intakes or assessments preferred.
• Experience working with vulnerable or underserved populations.
• Ability to interact effectively with clients, providers, and community organizations.
• Comfortable attending community events and working within the local service network.
• Basic Microsoft Excel and general computer skills.
