Primary ResponsibilitiesProvides high‑quality clinical services within the scope of practice as defined by state law, in accordance with the plan of care and in coordination with other health‑care team members. Follows all applicable infection control procedures.
Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record using the agency’s electronic visit verification program (as applicable).
Actively participates in the agency’s care‑coordination process, including timely reporting of patients’ needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physicians, other clinicians, agency management staff, and case coordinators.
Required Qualifications- Current and unrestricted Licensed Practical Nurse licensure in state of practice
- Current CPR certification
- Current driver’s license and vehicle insurance, or access to a dependable vehicle or public transportation
- Ability to function in any home setting regardless of age, race, creed, color, sex, disability, or financial condition of the client
- LA – Current LPN licensure in LA without restrictions
- 1+ years of experience working as an LPN
- Ability to work flexible hours as required to meet client needs
- Ability to manage multiple tasks simultaneously
- Ability to work independently
- Good communication, writing, and organizational skills
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, and other factors. In addition to your salary, you will have access to a comprehensive benefits package, incentive and recognition programs, and 401(k) contributions (subject to eligibility requirements). The hourly pay for this role ranges from $21.70 to $32.55 per hour based on full‑time employment.
