RN-Home Health Part Time

The RNField Clinician position will provide nursing care which includes:

• comprehensive health and psychosocial evaluation
• monitoring of patient's condition
• health promotion and prevention
• coordination of services
• promotion of patient/family independence

• Provide or supervise skilled nursing services in accordance with physician's orders
• Asses and regularly re-evaluate the nursing needs of the patient
• Develop, initiate, implement, and update the plan of care as needed or at lease every 60 days
• Initiate preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety
• Prepare clinical notes and OASIS assessments
• Coordinate services and inform physician and other personnel of changes in the patient's condition and needs
• Observe and report patient's response to treatment and any changes in his/her condition to the physician and supervising registered nurse
• Function as the patient advocate in all medical decisions affecting the patient
• Evaluate the effectiveness and outcomes of care
• Plan for the patient's discharge from home care services

• Must hold a current license issued by the Louisiana State Board of Registered Nurse Examiner's without restrictions
• Must have a minimum of one year experience as a registered nurse
• Current CPR certification
• Critical thinking skills, compassionate
• Home Health experience is preferred but not required
• ER or ICU experience is preferred but not required
