The Intake Coordinator is responsible for, but not limited to:
- Completing initial inquiry forms for all potential home health admissions and forwarding completed forms to the appropriate clinical staff, Director of Clinical Services, and/or Administrator.
- Working to convert qualified referrals into admissions whenever possible.
- Notifying the Administrator when referrals do not convert to admissions.
- Tracking all inquiries and referrals daily and providing a weekly report to the Administrator.

The Intake Coordinator is responsible for, but not limited to:
- Completing initial inquiry forms for all potential home health admissions and forwarding completed forms to the appropriate clinical staff, Director of Clinical Services, and/or Administrator.
- Working to convert qualified referrals into admissions whenever possible.
- Notifying the Administrator when referrals do not convert to admissions.
- Tracking all inquiries and referrals daily and providing a weekly report to the Administrator.
- Assisting patients, families, and responsible parties with completing admission paperwork as needed.
- Working collaboratively with appropriate staff to ensure all required written or verbal consents are obtained.

Education:- Associate degree in a related field preferred.
Experience:
- Five to eight years of related experience preferred.
- Experience in an intake, admissions, healthcare, or related capacity preferred.
- Valid driver’s license and reliable transportation required.
- Must be willing to travel.
- Excellent communication, interpersonal, and organizational skills.
