Shift Type: Full-time, typically 40 hours/week; daytime visits with flexible start/end times based on patient caseload (Monday-Friday typical; exact times vary)

Candidate Pay Rate: $100 - $70 per hour

• Perform comprehensive patient evaluations in the home setting to assess mobility, strength, balance, and functional needs
• Develop and implement individualized treatment plans that promote independence and improve quality of life
• Provide therapeutic exercises, gait training, balance training, and manual therapy as clinically appropriate
• Educate patients and caregivers on home exercise programs and safety techniques to support long-term outcomes
• Monitor and document patient progress in compliance with agency and regulatory standards
• Communicate effectively with physicians, nurses, and case managers to coordinate care
• Participate in care plan meetings and contribute clinical expertise to interdisciplinary teams

• Active New Jersey Physical Therapist license required
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Current CPR/BLS certification
• Minimum 1 year of clinical experience; home health experience preferred
• Reliable transportation and valid driver’s license for patient visits
• Strong time management and organizational skills

• Doctor of Physical Therapy (DPT) or equivalent — graduate of an accredited Physical Therapy program

• Physical Therapy evaluation and treatment planning
• Therapeutic exercises and gait training
• Balance training and manual therapy techniques
• Developing and teaching home exercise programs
• Patient education and safety techniques
• Effective communication with interdisciplinary teams
• Caseload management in a home health setting
