$60,000 base + $10,000–$15,000 commission

The Admissions & Outreach Coordinator will build and maintain referral relationships with hospitals, schools, crisis centers, and community partners. This role involves both field-based outreach and administrative responsibilities, ensuring families receive seamless support through the admissions process.

• Conduct 20–25 in-person outreach visits per week within a 50-minute radius
• Educate community providers on behavioral health programs and appropriate referral processes
• Generate referrals by maintaining strong relationships with community partners (patients are engaged only after referral)
• Handle inbound referral calls, gather intake information, and guide families through next steps with empathy
• Collaborate with clinical and administrative teams to ensure smooth intake and follow-up

• Strong communication and interpersonal skills
• Ability to build and maintain professional relationships
• Experience in outreach, case management, or a related field preferred
• Comfort with a field-based schedule
• Medical, dental, and vision coverage
