At Home Liaison-Home Care-FT – Meridian Home Care, Borough of Neptune City, New Jersey. Requisition # Shift: Day. Status:

Responsibilities- Represent the organization and introduce service lines and products; identify opportunities for cross-selling to meet patient needs.
- Gather external facilities patient information from charts and computer systems and forward to Central Intake (Hospital Liaison may assist in gathering/sending documentation).
- Deliver durable medical equipment (DME) and instruct patients/families in its use; assist in the face-to-face process by obtaining and sending documents.
- Gather specific patient information from charts and systems and forward to Central Intake for data entry.
- Provide information, answer questions, deliver collateral to educate and promote the organization; identify opportunities to cross-sell to patients/families.
- Develop ongoing relationships with case managers, social workers, physicians and staff; build relationships with referral sources to ensure seamless services.
- Ensure timely and appropriate communication with organization operations; use tracking systems to follow up on pending cases; own the delivery of services to meet customer and referral source expectations.
- Act as liaison between patient and the organization as the initial face-to-face representative; maintain exemplary customer service by addressing patient needs.
- Process DME (Hospitals Only); assist with collecting/sending DME documentation when requested; deliver DME and instruct users.
- Assist in obtaining/sending signed face-to-face forms.
- Other duties and projects as assigned; respond to departmental needs to maintain operations during staffing challenges.
- Adhere to HMH's organizational competencies and standards of behavior.
- Lifts a minimum of 10 lbs, pushes/pulls a minimum of 10 lbs, and stands a minimum of 4 hours per day.
Education,
Knowledge, Skills and Abilities Required:
- High School diploma, GED, or GED-equivalent programs.
- Minimum of one (1) year of customer service experience.
- Proficient in Microsoft Office Suite.
- Strong customer service skills.
Skills and Abilities
Preferred
- Homecare experience.
- Sales experience.
- Two (2) to four (4) years of healthcare experience.
Starting at $22.77 hourly.
