Responsibilities:

• Provides a comprehensive speech therapy evaluation.
• Documents findings on the standardized evaluation form neatly, accurately, and adhering to all guidelines.
• Provides a comprehensive treatment plan including long and short-term goals, frequency, duration, and treatment modalities.
• Ensures MD orders are obtained for evaluations, treatments, recertifications, and discharges.
• Provides comprehensive treatment to patients utilizing modalities and modifies patient treatment as indicated while adhering to precautions.
• Completes all documentation as necessary.

Qualifications:

• Graduate of an accredited university with an MA, or M.S. in communication disorders that is recognized by ASHA.
• Holds current license and/or registration in the state (as applicable).
