Position Overview:
We are looking for an experienced and compassionate Social Worker Director to oversee our social services department. This key role involves ensuring the delivery of high-quality, resident-centered care, coordinating with interdisciplinary teams, and supporting the emotional and psychosocial needs of our residents and their families.

Social Worker Director Key Responsibilities:

• Lead and supervise the social services team, providing guidance and support.
• Conduct assessments and develop care plans to address residents' emotional, psychological, and social needs.
• Collaborate with the medical team to ensure holistic care for residents.
• Advocate for residents' rights and facilitate communication between families, residents, and staff.
• Manage crisis interventions and provide counseling services as needed.
• Oversee compliance with federal and state regulations, ensuring social service standards are met.
• Provide support during discharge planning and transitions of care.

Social Worker Director Qualifications:

• Master’s Degree in Social Work (MSW) from an accredited program.
• Active Social Worker license in New Jersey (LCSW preferred).
• Experience in a healthcare setting
• Strong knowledge of state and federal regulations regarding social services in long-term care settings.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary team.
