Excelcare at Norwood is seeking a compassionate and organized
Social Worker to support our residents and social services department. This part-time role is essential to helping residents navigate care transitions, maintain emotional well-being, and access necessary resources. • Assist with resident admissions, discharges, and transfers
• Support residents and families with social, emotional, and practical needs
• Help coordinate care plans and communicate with interdisciplinary teams
• Maintain accurate documentation in compliance with state and federal regulations
• Participate in resident assessments and care conferences
• Provide support during grievance resolution and advocacy efforts
• Help organize resident programs and community resource referrals
• Maintain confidentiality and uphold resident rights at all times

• Experience in long-term care, social services, or healthcare settings a plus
• Strong communication and interpersonal skills
• Ability to manage multiple tasks and maintain accurate records
• Familiarity with EMR systems and HIPAA regulations
• Compassionate, professional, and team-oriented approach
• Reliable and punctual work habits
