We have an exciting opportunity for a Registered Dietitian to coordinate nutritional care for long-term and sub-acute care patients.
Responsibilities include:
Conducting nutrition assessments, participating in care plan meetings, providing education to patients and families, obtaining food preferences, and providing support.
Qualifications include:
Bachelor's degree; familiarity with DOH regulations/standards, the state survey process, electronic medical records, Microsoft Office, MDS 3.0, and knowledge of Geri-menu.
