The Registered Dietitian will be responsible for delivering high-quality nutritional care to residents, including:
Completing new admission, re-admission, quarterly, annual, and significant change assessments
Monitoring and documenting resident weights, weight changes, pressure ulcers, and wound care nutrition needs
Managing nutritional care for hemodialysis and enteral feeding residents
Completing MDS documentation
Updating care plans and participating in interdisciplinary care plan meetings
Reviewing and updating resident meal and snack preferences, entering diet orders, and assisting with menu planning as needed
Conducting meal rounds and nutrition audits
Providing nutrition education and counseling to residents and families
Attending morning clinical meetings as required
Maintaining strong communication with physicians, nurse practitioners, nursing staff, and the interdisciplinary care team
Performing additional related duties as assigned
Active Registered Dietitian license in the state of New Jersey required
Long-term care or short-term rehabilitation experience preferred
Experience with Point Click Care EMR preferred
Experience with Meal Tracker preferred
Ability to make independent clinical decisions
Strong understanding of nutritional service regulations, standards, and best practices within long-term care settings
Excellent verbal and written communication skills
