Bold Age PACE is seeking a clinician to deliver primary care and continuous health services to our participants, coordinating with the clinical care team to assess needs and develop care plans in alignment with PACE standards.
You will perform comprehensive history and physicals, monitor medications, work with the IDT to coordinate referrals, and participate in 24/7 on-call coverage while supporting safe transitions and quality outcomes for frail older adults served at home and in our centers.
