VA Careers - Licensed Clinical Social Worker
DutiesDuties include but are not limited to the following:
- Provide clinical evidence-based services and interventions
- Support program development
- Provide caregiver, Veteran, and staff education on caregiver issues
- Conduct community outreach
- Support resource development
- Participate in continuous quality improvement activities
- Provide evaluation/consultation
- May have supervisory duties assigned under the direction of the Caregiver Support Program Manager.
- Assists in developing the strategic plan and implementing policies and procedures for local expansion of the Program of Comprehensive Assistance for Family Caregivers (PCAFC)
- Prepares reports and statistics for facility, VISN, and national use, in collaboration with the Caregiver Support Program Manager
- Develops and implements systems to track service activities, including visits, referrals, caregiver demographics, equipment utilization, and caregiver and Veteran satisfaction
- Administers information and analytical systems to evaluate and enhance the quality of service provided to caregivers, families, and Veterans
- Responsible for meeting performance metrics for the assigned area
- Develops, implements, and evaluates appropriate utilization of services, as assigned
- Conducts caregiver and family clinical interventions to identify caregiver stress, psychiatric illness, emotional distress, and social needs as treatment issues
- Applies professional judgment, including knowledge of normal and abnormal behavior, in daily interactions with caregivers and the Veterans they care for
- Provides referral to VA medical and mental health treatment and other services, including VA benefits, as well as community-based social services and non-VA entitlement programs
- Practice independently, exercising initiative and independent judgment in day-to-day activities
- Receives supervision/consultation of an administrative and professional nature, with the supervisor providing guidelines on general policy and direction of Social Work or the relevant area of the Caregiver Support Program
- Works collaboratively with the Caregiver Support Program Manager, TCM Case Managers, primary care providers, and RN Care Managers assigned to the program
- Directs assignments of Home Visit staff and may supervise other professionals related to cases and areas of assignment
- Utilizes and assists in maintaining a data monitoring system to measure program efficacy
- Assist in collecting and analyzing local data for reports with the Caregiver Support Program Manager, Chief of Social Work Service, VISN Caregiver Support Program Manager, and the National Caregiver Support Program, Care Management and Social Work Service, Office of Patient Care Services
- Evaluates program progress and outcomes and uses data to continuously modify and improve the program and processes
- Develops resources to further enhance the general caregiver support program to meet the unique needs of caregivers in the catchment area
- Perform other assigned duties.
*** Must possess a valid driver's license to travel to homes of Veterans and families throughout VA NJ HCS catchment areas for assessments and interventions as needed limitedly.
