Physical Therapist – Mobile Outpatient Therapy

Full‑time, salaried position providing mobile outpatient therapy for older adults in the home and independent living settings. The role includes evaluating, treating and coordinating comprehensive care for patients in assisted, independent, and community places. An $5,000 bonus is offered.

Posted Salary Range: USD $80,000.00 - USD $ /Yr. Additional ranges: USD $79, - USD $95, /Yr.; USD $87,802.00 - USD $ /Yr.; USD $40.00 - USD $52.00 /Hr.;
Total Compensation Range: $82,000 - $115,000 / Year.

Responsibilities include:

• Provide outpatient rehabilitation services in home and independent living settings.
• Evaluate, treat, and coordinate comprehensive care for older adult patients.
• Develop individualized physical therapy programs and plan of care.
• Implement therapy programs, including therapeutic exercise, manual therapy, airway clearance, and other modalities.
• Educate and train patients and caregivers in skills for independence and productivity.
• Maintain safe environment, eliminate fire and safety hazards.
• Supervise Physical Therapy Assistants and support personnel.
• Ensure adherence to department and patient treatment schedules, timely clinical and billing documentation.
• Attend and contribute to patient care, staffing conferences, and related meetings.
• Make referrals to other disciplines and agencies.
• Adapt program and/or plan of care as needed.
• Order and maintain supplies and equipment.
• Pursue patient service excellence and highest quality care.
• Screen, examine, evaluate patients, establish diagnoses, determine treatment goals, design and plan of care.
• Perform functional, manual, airway, integumentary, electrotherapeutic, and wellness interventions.
• Conduct home and community mobility assessments and provide recommendations.

Qualifications:

• Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy, or a related field plus a Bachelor's degree, or Bachelor's degree plus 5 years of progressively responsible PT experience.
• Foreign Physical Therapy degree evaluated as U.S. equivalent.
• Valid driver's license or reliable, independent transportation.
