This role is primarily onsite at assisted living facilities, which provides a unique work setting away from traditional clinic walls and waiting rooms. Instead, you will be interacting with patients in their living spaces, allowing you to build rapport and trust. Your typical schedule will be Monday to Friday from 9 AM to 5 PM, fostering a balanced work-life dynamic while fulfilling essential therapy needs within a community-oriented environment.

• Implement tailored treatment plans: Work closely with licensed physical therapists to carry out customized rehab programs for residents, focusing on orthopedic recovery and enhancing mobility.
• Deliver empathetic care: Provide treatment with a deep sense of compassion and attention to detail, ensuring that each patient feels valued and understood throughout their rehabilitation journey.
• Monitor patient progress: Regularly assess and document each patient's response to treatment, making adjustments as necessary to align with their rehab goals.
• Team collaboration: Work as part of a multidisciplinary clinical team, collaborating with physicians, occupational therapists, and nursing staff to ensure coordinated care for each resident.
• Build lasting relationships: Establish trust and rapport with residents, caregivers, and facility staff, recognizing the importance of a supportive community.
• Maintain accurate documentation: Utilize a convenient mobile Electronic Medical Record (EMR) system for accurate patient charting, ensuring up-to-date and organized records that enhance care coordination.

You will primarily work with elderly residents in assisted living facilities, focusing on individuals who require assistance with mobility and daily activities. This role demands a caring and empathetic approach, as you will be directly affecting the lives of seniors striving to maintain their independence and dignity in a supportive environment.

• Licensure: Must possess an active Physical Therapist Assistant license from the State of New Jersey.
• Education: A Bachelor's degree in a related field is required to ensure a foundational understanding of physical therapy principles.
• Skills: Strong communication abilities, time management skills, and adaptability to handle diverse clinical situations.
• Teamwork: Ability to work independently while effectively being part of a cohesive interdisciplinary team.

• Two or more years of prior experience in outpatient settings, geriatrics, home care, or skilled nursing facilities (SNF) is preferred.
• Demonstrated passion for improving the lives of seniors through rehabilitation and promoting their independence and dignity.

The position offers a competitive salary range between $60,000 to $75,000 annually, commensurate with experience and qualifications.
