PAY RATE
: $21.15 per hour

Job Summary
  • Teach independent living skills and assist with/provide consumers with personal care.
  • Arrange/provide transportation for consumers.
  • Assist consumers with recreation/inclusion, plan vacations.
  • Administer, monitor and record medication distribution according to policy. Review and ensure completion of all medical logs.
  • Coordinate all medical appointments, ensure all related documentation is accurate and complete, attend appointments when needed.
  • Ensure all medical recommendations and follow up are communicated and completed in a timely fashion.
  • Coordinate/provide all necessary agency and intra-program staff training; provide staff with all necessary information needed to effectively carry out their job duties.
  • Ensure proper communication of all consumer issues/needs to all necessary parties; proactively teach/role model/guide staff on how to appropriately work with and interact with consumers, other staff, families, and guardians.
  • Address staff concerns as needed; encourage staff to share ideas and suggestions.
  • Attend all staff meetings, and other consumer-related meetings.
  • Ensure staff, training, and beeper schedules are received and posted.
  • Complete and submit all required daily, weekly, bi-weekly, monthly, quarterly, and annual reports as scheduled.
  • Using Therap to do logs
  • Handle and document all financial transactions for consumer accounts in accordance with agency and DDD policy; reconcile all consumer accounts monthly.
  • Ensure preventative maintenance of the program/vehicles(s) including regular vehicle service and inspections.
  • Perform other duties as required by the nature of the position, or as requested by supervisor.

EDUCATION
:
High School Diploma or equivalent required. Undergraduate degree in relevant field preferred.
