The Food Access Coordinator is responsible for conducting day-to
-day operations of the RWJBarnabas Health Food Farmacy project at Monmouth Medical Center. You will enroll participants, manage food pantry distributions and help to meet grant deliverable requirements for the program. You will work with our partners at Ful Fill Food Bank of Monmouth and Ocean Counties and additional farm partners to receive and maintain food pantry inventory.
- Registering Food Farmacy patients who are referred by their healthcare provider and completing all necessary enrollment paperwor k
- Managing food pantry distribution hours: packing client bags and documenting visits
- Conducting in-person or phone pre and post program intervention surveys with patients,
- Receiving and stocking food pantry inventory from Fulfill and other community partner organizations
- Collecting enrollment and attendance data for monthly departmental and annual community benefit reporting
- Assisting with community partner billing and Food Farmacy departmental administrative tasks as assigned,
- Collaborating with MMC s assigned SNAP Navigator to ensure comprehensive food access resources are available for all patients,
- You communicate clearly and compassionately with patients, families and healthcare professionals
- You have awareness and comprehension of food security and food access issues
- You stay organized and can effectively manage multiple priorities
- You enjoy coordinating schedules and solving challenges
- You are detailed-oriented
- You respond in timely manner to all office and schedule inquiries
- You work collaboratively with team members
- You are comfortable using electronic medical systems, including EPIC
To be considered for this opportunity, you must have a bachelor's degree, preferably in Nutrition and Dietetics or Public Health. Knowledgeable with Microsoft Office and Epic or other medical record system. Bilingual skills are a plus.
