Road Warrior, JOB FUNCTION:
To provide services to pregnant women including referrals, education, monitoring of pregnancy and on-going support during pregnancy as well as during the first months of life, per Head Start regulations.

ESSENTIAL FUNCTIONS:

Work closely with the Home Visiting Manager management and staff to ensure that all Health Services for Early Head Start Expectant Families and infants 1 year or younger transitioning from the Expectant Families program are delivered effectively and efficiently:
Assist Family Partnership staff to maintain accurate, confidential up to date health, dental and nutrition records for all EHS children coming from the Expectant Families program during the child’s first six months in the center-based EHS program as outlined in the Procedure Manual in compliance with Head Start Performance Standards, New Jersey Licensing and HIPPA regulations. Monitors and documents health/nutrition status of all EHS children coming from the Expectant Families program during the child’s first six months in the center-based EHS program in the program to ensure all screenings, assessments, immunizations are up to date and in compliance.

Performs pertinent screenings for all EHS children coming from the Expectant Families program during the child’s first six months in the center-based EHS program to remain in compliance with Head Start Performance Standards In collaboration with family workers and Child/Family Site Director reviews family intake materials and health updates for health/nutrition needs for children transitioning from Expectant Mothers program to EHS center based.

When appropriate, completes referrals for health, dental and nutrition issues of children; seeking outside resources as appropriate for children transitioning from Expectant Mothers program to EHS center based. Collaborate with Family Partnership staff to ensure that all children, families receive the required medical, dental and nutrition assessments and follow-up within the time requirements of Head Start Performance Standards and N.J. Licensing, for children transitioning from Expectant Mothers program to EHS center based.

Advocates preventative health, dental, and nutrition care for all clients through established channels for available services to Head Start/Early Head Start families. Involved in regular, purposeful home visits for families or enrolled children as necessary. Direct Services for EHS Expectant Parents program:
Acts as an advocate and liaison between Expectant Parents and service providers. Regular home visits with Expecting Parents according to Head Start Procedure. Completes Community Family Partnership Agreement with all Expecting Parents, making referrals to community resources as needed with assistance from Community Family partnership staff. Assists Expecting Parents with transition from the Expecting Parents program to child care as per parent’s preference within three months of child’s birth.

Assists Expecting Parents obtain medical, dental, nutrition services per Head Start Performance Standards using established Community and Head Start resources. Addresses special dietary needs and/or requirements for EHS infants 1 year or younger for children transitioning from Expectant Mothers program to EHS center based. Coordinates with other social service agencies and community resources for EHS Expectant Families or for children transitioning from Expectant Mothers program to EHS center based.
