• Provide certified home health care services to clients in their homes or in our community-based settings
• Conduct initial assessments and develop care plans tailored to each client's needs
• Deliver skilled nursing services, including wound care, medication management, and IV therapy
• Collaborate with other healthcare professionals to ensure seamless care transitions
• Document client progress, identify areas for improvement, and implement changes as needed

• Certified Home Health Nurse (ASAP certification preferred)
• Active NJ State licensure
• 0 years of experience in home health care (training provided)
• Strong communication and interpersonal skills
