Registered Nurse Care Manager - Full Time - Homecare

Elder Care Homecare is seeking a New Jersey Registered Nurse to join our company's team in Bergen County. We are a private pay homecare agency servicing NY, NJ, CT, and MA, with additional expansion planned within the next year. This role is based in our Saddle Brook office.

We have been in operation for over eight years, however our growth has dramatically increased over the last three. The ideal candidate can effectively build relationships, has excellent communication and organizational skills, and is comfortable in a leadership/training role. Shift and schedule:
Weekends as needed

Pay: $100,000 - $120,000 per year

• Oversee clientele that is assigned to your care
• Create and maintain relationships with our caregivers and nurses caring for your client
• Execute, implement, and update the plan of care for the client, ensuring the caregiver is aware of changes
• Spend 75% of your time in home visits
• Onboard prospective clients
• Monitor discharge from hospital, SNF, or rehab
• Complete administrative paperwork in accordance with our policy and DOH

• Registered Nurse with up-to-date license
• Ability to work autonomously
• Availability for some weekends throughout the month
• Comfortable clinically training RNs and caregivers on-site for their duties
