A-Line Staffing is now hiring a Full-Time, Field Based, Hospice Nurse in Bernards, NJ!
Job Summary We are seeking a compassionate and experienced Registered Nurse (RN) to join our Hospice team. The RN will provide skilled nursing care to hospice patients in their homes, ensuring high-quality, patient-centered care while supporting patients and families through end-of-life services. Hospice Nurse Compensation The pay for this position is $58/hr + mileage Benefits are available to full-time employees after 90 days of employment A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Hospice Nurse Highlights This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs The required availability for this position is Full-time: 5 days per week
Shift: 8:00 AM – 4:00 PM (7.5-hour shifts) Weekend/On-call:
Required (1 weekend per month) Travel:
Required; may float to different counties based on needs Hospice Nurse Responsibilities
· Perform comprehensive patient assessments to determine hospice eligibility, needs, and progress toward goals
· Develop, implement, evaluate, and revise individualized plans of care for patients and families
· Ensure appropriate utilization of hospice services and maintain visit productivity standards
· Complete admission, re-visit, and discharge assessments within required time frames, including timely electronic documentation transfers
· Provide patient and family education using age-appropriate teaching methods and revise teaching plans as needed
· Coordinate case management activities, including prioritizing patient schedules and documenting care activities
· Demonstrate knowledge of hospice regulations, insurance reimbursement guidelines, and interdisciplinary care coordination
· Participate in Interdisciplinary Team (IDT) meetings every two weeks and communicate patient progress toward goals
· Supervise Home Health Aides (HHA) according to agency standards
· Maintain medication safety, including reconciliation and ensuring current medication orders
· Follow infection control standards and maintain a safe environment for patients and staff
· Participate in Performance Improvement (PI) activities, UR meetings, and quality initiatives
· Provide orientation and mentorship to new staff and assist with cross-training initiatives
· Support agency growth through flexibility, teamwork, and acceptance of additional assignments as needed Hospice Nurse Requirements
· Active Registered Nurse (RN) license in the state of New Jersey
· Associate Degree in Nursing (ASN) OR Nursing Diploma
· Minimum of 2 years of RN experience in a home hospice setting
· Valid New Jersey driver’s license
· Current Basic Life Support (BLS) certification
· Experience using Electronic Medical Records (EMR) systems
