Position: LPN Floor Nurse 3 to 11pm 5K Sign ON BONUS

Responsibilities
  • Direct daily activities of licensed practical/vocational nurses and certified nursing assistants in accordance with federal, state, and local regulations and established facility policies and procedures.
  • Follow the nursing model to promote holistic care for residents.
  • Ensure nursing personnel comply with policies and procedures.
  • Participate in admissions, discharges, and transfers of residents as required.
  • Assess changes in residents’ status, notify physicians and families, and document accordingly.
  • Transcribe physician orders to the medical record and carry out orders as written.
  • Prepare and administer medications per physicians’ orders and observe for adverse effects.
  • Perform wound treatments per physicians’ orders, observe for changes, and document accordingly.
  • Schedule follow‑up appointments and transportation needs as indicated.
  • Provide nursing leadership to unit staff.
  • Provide direct care skills (colostomy changes, tube feedings, wound care, suctioning, IV administration, etc.) in accordance with policies.
  • Perform rounds to ensure resident needs are met and personnel perform assigned duties.
  • Collaborate with interdisciplinary team members to meet residents’ holistic needs.
  • Participate in management team meetings to discuss census changes, resident status changes, or care needs, complaints, or concerns.
  • Assist in evaluating nursing personnel performance and implement discipline per policies.
  • Inform appropriate departments of concerns or complaints and file grievances as necessary.
  • Report incidents or unusual occurrences to the unit manager, assistant director, or nursing director and participate in investigations.
  • Participate in end‑of‑life care as required.
  • Ensure adequate stock of medications, supplies, and equipment, and notify personnel of needs.
  • Initiate, review, and update care plans as required.
  • Promote teamwork, mutual respect, and effective communication.
  • Participate in survey processes during inspections.
  • May participate in QAPI activities as needed or assigned.
  • Ensure infection control principles are followed and correct deficiencies in practice.
  • Perform other duties as assigned.
