MDS Coordinator, RN

We are seeking a skilled MDS Coordinator, RN to work in our Cape May facility. This role supports the delivery of high-quality care and ensures proper documentation and reimbursement for patient services.

Responsibilities- Participate in the Patient Assessment Process through completion of MDS and Care Area Assessments (CAAs).
- Develop individualized patient care plans, identify patient needs, and project outcomes per federal and state regulations.
- Contribute to the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring accurate completion, computer encoding, and electronic transmission of the MDS.
- Identify changes in patient condition with the interdisciplinary team and coordinate scheduling and completion of indicated assessments.
- Assist with Activities of Daily Living training for new hires and provide continuing education to existing staff.
- Attend educational programs to maintain knowledge of current RAI/Prospective Payment System regulations.
- Registered Nurse, currently licensed by the state and in good standing.
- Prior experience in a hospital, long‑term care facility, or related healthcare setting.
- Experience completing MDSs and Care Plans.
- Knowledge of general, rehabilitative, and restorative nursing practices, as well as laws, regulations, and guidelines governing long‑term care.
