Responsibilities for LPN
- Licensed Practical Nurse:

• Assist with resident assessment and implementation of care needs and will participate in resident and family care conference

• Must be able to effectively communicate with residents, families, staff, physicians, outside providers and the public

• Ensures that changes in resident condition are evaluated, monitored, documented and effective interventions are implemented

• Provide support and assistance to medication management systems

• Ensure that medications are dispensed in a safe and effective manner, keep medication cart orderly, and all medications appropriately packaged and stored

• Monitor resident health concerns including weight and nutrition, skin integrity, mobility, fall management and pain management

Requirements for LPN
- Licensed Practical Nurse/RN
- Registered Nurse:

• Completion of an accredited school of practical nursing/Registered Nursing

• Current licensure as a Practical Nurse ( LPN )/Registered Nurse (RN) in the State of New Jersey
