We are seeking a Per Diem Nurse Practitioner to work with hospice patients in Essex County. This position is per diem, with no guaranteed hours, and the Nurse Practitioner is paid $125 per client visit.
Responsibilities- Perform patient medical examinations and palliative care visits for face‑to‑face encounters required to certify patient eligibility for Medicare Hospice Benefit after six months of hospice care.
- Document visits in the patient’s clinical record and required encounter forms.
- Provide medical care services and palliative care interventions on an as‑needed basis.
- Communicate effectively with the Medical Director, Administrator, and the Hospice Interdisciplinary Team.
- Operate in compliance with prescribed policies, procedures, and all applicable state and federal regulations.
- Adhere to Hospice bylaws, operational policies, billing procedures, Medicare Conditions of Participation, and New Jersey licensure guidelines.
- Maintain a professional, compassionate attitude and interact positively with patients, caregivers, staff, and the interdisciplinary group.
- Graduation from a master’s‑level program designed to prepare nurse practitioners or clinical nurse specialists.
- Completion of a graduate‑level 3‑credit pharmacology course or at least 45 hours of pharmacology integrated into the master’s program.
- National certification as an Advance Practice Nurse in New Jersey (NJ Board of Nursing); currently licensed and certified (N.J.A.C. 13:37-7.1).
- Valid driver’s license and reliable, insured transportation in accordance with New Jersey requirements.
- Minimum of two (2) years’ experience as a Nurse Practitioner.
- Understanding of the hospice program of care.
- Timely and professional service delivery in compliance with hospice bylaws and operational policies.
- Positive and professional demeanor; efficient time management; teamwork; role model.
- Effective communication; courteous and cooperative with patients, caregivers, colleagues, supervisors, and others.
- Optimism, enthusiasm, and willingness to work constructively with health‑care team members.
- Supportive of the mission and vision of the organization.
